ADMISSIONS PROCESS
De La Salle Academy recently announced changes to its admissions process. We will no longer be taking 5th grade applications.

STEP ONE: APPLY
Once you have completed the application, please submit the following admissions documents (some of which can be found here) to the school:
Recent photo of the student
Copies of 2021 tax return and W-2's/1099
Birth certificate
Baptismal certificate (if applicable)
Copy of most recent report card
If you do not submit all required documents, your application will remain incomplete. Incomplete applications are not considered for admissions.
To submit admissions documents to the school applicant families can drop off in person or mail them to the school directly:

STEP TWO: ADMISSIONS TESTING
Qualified applicants are REQUIRED to participate in a test administered by DLSA. Families will be contacted to setup the admissions testing on an individual basis.
STEP THREE: FAMILY INTERVIEW
Students that make it to this step will have an interview, during which two DLSA staff members will meet with the student and at least one guardian. Both the family and the student will have separate questions during the interview. The school will reach out to the family to schedule the visit.
